Why programs should address social determinants of health
CHW programs provide assistance to patients in accessing primary care or preventive services, especially in underserved communities18. Many CHW programs focus on people who are experiencing diabetes1-11, cardiovascular disease1,12,13, obesity8, or require chronic disease management14 and cancer screening15-17. In addition, CHW programs focus on social determinants of health — such as school quality, housing affordability, income, working conditions, community safety, and environmental factors — that can have profound effects on health and well-being. By targeting these factors and connecting patients to affordable health care, community health workers have the power to dramatically improve the lives of their clients.
